Activities Request worksheet for use of Club Facilities
The information with the committee’s recommendations will be presented to the Board for
approval and entered on the club calendar. New requests should be sent to the committee prior
to a board meeting and preferably 30 days prior to date. The requesting member may also
address the Board and is responsible to notify the club of any changes.
